• A storage case for trays;
• One free syringe of whitening gel at on-time scheduled regular cleaning/exam appointments. ($30value).

Provisions/Requirements:

• Each new patient will need to have a New Patient Exam, appropriate x-rays and cleaning. 
• Existing patients with previous Whitening Treatment from our office, if all other criteria are met, are eligible immediately for a free syringe.
• Patient must be free from active decay (cavities) and periodontal/gum disease. If tooth decay and/or gum disease is present, there must be plans for treatment at our office. 
• Medical/dental issues may prevent patient from being a candidate for teeth whitening. 
• Patient’s billing account(s) with our office must be current. 
• Patient must have at least two regular dental cleaning/exams with appropriate radiographs.
